Please fill out the form below in order to become a licensed SFLA group and choose your FREE resources!! First-year signers will receive a custom Students for Life banner for your group.

Make sure to fill out the shipping information so we know where to send your items! We are also unable to ship your items until your licensing agreement has been signed and received!
